PLEASE READ THESE INSTRUCTIONS IN THEIR ENTIRETY BEFORE COMPLETING THE ONLINE FORM

Annual Agency and Program Reports Due By:  July 31st at 4:00 p.m.  

SUBMIT THESE ITEMS by the deadline date and time:

• completed online form for your agency;
• completed Agency signature form for your agency;
• completed online form for each program; and 
• completed Excel workbook for each program.

The individual Excel workbook(s) have been sent to the Agency Executive Director via email. They need to be completed and uploaded as part of this form. 

BE AWARE THAT:

• The system allows for one report at a time to be filled out by each User Account (unique email address and password).
• Prior to submission, the system holds one draft of a Report form per user.
• Once one report is submitted, a new blank form will appear after the Guidelines section. That user can then submit any subsequent reports, if applicable. 
• Alternately, you can create a separate User Account for each report to be submitted.
  

TIP: Create a Word or Notepad document on your computer where you can draft the answers to the questions, then copy and paste them into the form when you are ready to submit. This provides a backup in case your information gets lost during the process of filling in the form.
